For referral sources

A clear next step for every referral conversation.

Start with the service, general location, timing, and the person or team we should call back. A&A will explain the current process and secure information channel.

Who can call

One front door for different care partners.

A general referral conversation can begin with any member of the person’s care or support network. Authorization, consent, eligibility, orders, and program requirements are confirmed through the appropriate process.

What to have ready

A faster conversation starts with five basics.

01

Service location

City, ZIP code, and the setting where support is needed.

02

General care need

The service being considered and what has changed.

03

Timing

Requested start date, schedule, discharge date, or appointment time.

04

Program or payer

Plan, waiver, authorization, or provider-order information already known.

05

Callback contact

The best person and number for the next step.

Privacy comes first

Use the public form for a callback—not for records.

Do not put diagnoses, medication lists, medical record numbers, insurance IDs, Social Security numbers, or emergency information in the website form or general email. Call A&A and ask which secure channel is appropriate.
