Start with the waiver—not a service list

Ohio DODD administers the Individual Options, Level One, and Self-Empowered Life Funding waivers. Each pathway has its own rules, budget structure, and service options, so the name of a service alone does not determine whether it is available.

A person's county board, service and support administrator, and individual service plan help shape what is authorized. A provider can explain its current capacity, but it cannot replace the eligibility or authorization process.

What to have ready for a provider conversation

• The waiver and services currently authorized
• The person's county and general service location
• The goals or routines the support should address
• Preferred schedule and requested start date
• Contact information for the service and support administrator
